Where to register for pregnancy?
First of all, the future mother needs to register for pregnancy with an obstetrician-gynecologist. Where to observe: in the women's clinic, commercial medical center, in the medical center at the hospital, you decide. It all depends on your preferences and material possibilities.
It can be observed for free in the women's consultation at the place of residence or at the place of actual residence, regardless of registration. To register for pregnancy in a women's consultation you need to show a passport and a policy of compulsory medical insurance. The presence of the policy, regardless of the place of its issuance, allows the patient to present it in any women's consultation or maternity hospital, where he must be recognized as valid, - this provides a unified system of compulsory health insurance. There are normative acts confirming the right to state medical care irrespective of the place of residence. In the absence of a policy, you will be given only emergency medical care.
It is better to register for the women's consultation, in which you have been observed for several years, so as not to violate the continuity of medical supervision. After all, there is all the data about your health, the diagnoses, the results of the examinations, treatment, etc. are recorded. In a new place, all this is missing, so some diseases remain unnoticed. It is necessary to be guided and to the place of observation: it is good, if it is convenient to get to the consultation. The cost of contracts is different: from 10 to 60 thousand rubles - it all depends on the amount of examinations, consultations of doctors, the period of pregnancy, etc. It is necessary to find out if the medical center in which you are going to be observed has permission to issue an exchange card, a license for certain types of obstetric care is not a guarantee of the existence of such a permit. Meanwhile, the exchange card is a document where the results of all the examinations conducted during pregnancy are recorded and it is necessary for admission to the maternity hospital. A woman who does not have an exchange card can only take in the observatory ward of the maternity home, where there are unopened patients and women with various infectious diseases. The woman receives the exchange card after the 28th week of pregnancy. Specify, whether the commercial medical center gives out to the clients the sheets of invalidity on pregnancy and sorts (easier speaking, whether it is possible to issue there hospital and maternity leave). The order of communication with an obstetrician-gynecologist will be built according to a scheme similar to that of a woman's consultation.
Another option for monitoring pregnancy is observation in the medical center at the hospital; its advantage is the possibility of conducting pregnancy and childbirth by an obstetrician-gynecologist.
When to register for pregnancy?
To register with a specialist should be in the early stages of pregnancy (up to 12 weeks), as this increases the likelihood of successful pregnancy and childbirth, and hence the birth of a healthy child. At this time it is advisable to begin examinations with specialists, taking tests to exclude all possible troubles and timely start treatment if one or another pathology is detected. Obstetrician-gynecologist will be able to systematically monitor your health, prescribe certain examinations at every stage of pregnancy to make sure there are no complications. In addition, our state pays to all women who have registered for pregnancy for up to 12 weeks, when they go on maternity leave, a one-time benefit of half of the minimum wage.
What do they do in the women's consultation?
On the first admission, the obstetrician-gynecologist will ask you about everything related to your health, weigh it, measure the pelvis with a tazometer (a special instrument that looks like a large compass with blunt ends, with which you can find out the pelvic dimensions), inspect the chair, take a smear from of the vagina. All this information and research is necessary in order to make a plan for managing pregnancy and to assume what difficulties can arise during childbirth. The doctor will give recommendations on rational nutrition, drugs that need to be taken, will write directions for tests and consultations. Most often blood tests are given on an empty stomach, you can find out in advance with your doctor whether it is necessary to comply with this condition during the next study.
There is a standard for medical examinations in pregnancy, which are free of charge. These are general blood tests (blood from the finger is taken), biochemical blood tests (blood from the vein is taken), blood test for HIV, hepatitis B, syphilis, general urine tests, vaginal smears, ultrasound examinations, dentist, ophthalmologist, therapist consultations. In the presence of chronic diseases of the mother, additional examinations and consultations may be necessary. If you have a need to do tests that are not included in the standard, they will have to be paid for. In most medical institutions, it is customary to skip pregnant women out of turn, so correctly indicate the remaining patients to their situation.
Before meeting with an obstetrician-gynecologist, you can compose an approximate list of questions that you need to ask in order to forget nothing and discuss all the details you are interested in. If the doctor of a woman's consultation detects significant abnormalities during pregnancy, there is always an opportunity to send the patient to a consultation with a larger medical center. For example, if such a consultation is in the Moscow Region, the patient may be referred for examination to the Moscow Regional Research Institute of Obstetrics and Gynecology.
Maternity leave: how much?
The disability certificate for pregnancy and childbirth is issued by a doctor as an obstetrician-gynecologist, and in his absence - by a general practitioner for the following periods:
in the absence of complications - from 3 weeks of pregnancy, the duration of 140 calendar days (70 days before delivery and 70 days after birth);
at multiple pregnancies -28 weeks of pregnancy for 180 days:
with complicated births - in addition for 16 calendar days. In these cases, the total duration of antenatal and postnatal leave is 156 calendar days (70 + 16 + 70).
